Jeremy “Jake” Danial Forman died June 11, 2019 at Frannie, Wyo.
Jake was born May 4, 1988 in Miles City, MT to Jerry and Becky Rainey Forman. He was immediately ready for any adventure he could find. Even when he was small, he was going full speed, whether it was running into the refrigerator with a snowmobile helmet on or jumping off the side of a pickup, trying to soar into a tree. As he grew older, he was always on his bike or a snowmobile. His two favorite things. He began welding at age 12, first with his dad, then with his step-dad at RZ Welding. He became well known as an expert welder before his 18th birthday. He graduated from Custer County District High School and went on to weld around Miles City and Billings. He learned to drive truck from his dad, got his CDL, and was enjoying a career in trucking. He made many mistakes and explained with a smile, “I have to learn things the hard way.”
His life changed forever when his son Zeke was born on Nov. 10, 2009 to Tara Losinski. Zeke and Jake formed a solid bond that will last forever. Exactly nine years after Zeke’s birthday, Nov. 10, 2018, Talon was born to Dawn Davis. Even before Talon was born, Jake considered him as his son and began raising him as such. At the time of his death, Dawn and his two sons were Jake’s world.
Jake is survived by his sons Zeke of Jordan MT and Talon of Billings MT; Dawn of Billings; his parents Jerry and Elaine Forman of Miles City, and Becky and Rick Zawada of Soldotna, Alaska; his sisters Amanda Forman, her partner Chuck Sluzenski and her daughter Rian of Windham, New Hampshire, and Erika Swanson, her partner James Davis and their daughters Claire and Elise of Miles City; brother Justin “Junior” Zawada of Soldotna, Alaska; grandmothers Helen Swanson of Miles City and Vi Zawada of Soldotna, Alaska; numerous aunts and uncles and cousins.
He was preceded in death by grandparents Ralph and Lainie Forman, Florence and Elray Rainey, Bob Swanson and Joe Zawada, and an uncle Mark Forman.
Family will receive friends on Saturday, June 15, 2019 from 2:00-4:00 p.m. at Stevenson & Sons Funeral Home in Miles City with a prayer service to follow at 4:00 p.m.

Forman famiy… my heart is breaking for all of you with the loss of Jake. Life will never be the same without your loved one, however, may your memories sustain you through your grief and the days ahead. I have the best memory of a cute, shined up little boy heading off to Kindergarten for his first day when he come to get his dad to go with them on the adventure. For the next ten years, I got to watch Jake grow into a young man. He was always very polite and greeted me with a smile whenever we met. I am hoping that Jake has finally found his peace… hugs to his family as he is laid to rest.

Jerry, Becky, Amanada, Zeke and Dawn I’m so sorry for ya’alls loss. Jake was my my best friend and I have so many good memories with him. He was such a good friend, son, brother, boyfriend and father with a huge heart. He was doing so good in life recently and making big strides to be the best he could be making us all proud! He was such a great dad to Zeke and Talon. He was so proud to be dad to his boys and how good of a person Zeke was turning out to be. I remember how he would light up when he would talk about Zeke or when he was headed to see him or even how happy he was after spending the weekend with him! Jake had SO many good qualities I could go on and on. I always wanted him to teach me how to weld like a pro! Jake is going to be missed by so many people and if there is anything you all need don’t hesitate to ask may you all have peace and comfort while going through this heart breaking time.

To the family, my most heart felt condolences. I have so many wonderful memories of Jake. Paint ball and BB gun wars, laughing endlessly for hours about some of the most ridiculous things, fishing and road trips. The list could go on. Jake was a person who helped me get through some of the toughest times in my life. He was one of the first people to hold my first born son. He was always there for me no matter the time of day or night. It completely breaks my heart that you are gone. This town holds so many memories of you.
